Information required prior to scheduling Intake:

  • Picture of the front & back of the adult client's insurance card(s) (if applicable)

  • Information about additional funding sources (if applicable)

  • Copy of guardianship paperwork (if applicable)

Our usual platform is down, so please email the above documents to You will then be emailed an Adobe Sign version of our Adult Services Request form.

Once that form is completed, it will automatically be returned to the Client Coordination Assistant, and you will be manually entered into our system.

The Client Coordination Director will contact you within 24-48 business hours of being entered into our system.
