    Information required prior to scheduling Intake    

Please complete the form for requesting Adult Client Services (see button below). If the client is not their own guardian, we will need a copy of the Guardianship Paperwork.​

 

Additional paperwork may be needed, depending on funding source.

We will reach out to you within 1-2 weeks after we receive the request.
