Paperwork Required Prior To Scheduling ABA Intake
For Returning ABA and Adult Clients
The following paperwork is needed to be emailed to clientservices@sonnenbergllc.com in order to request services for returning clients:

Copy of Primary insurance card, front and back

Secondary insurance card (if applicable), front and back
Once you have requested services, we will reach out to you and may request additional paperwork be sent via email or fax.
We will reach out to you within 1-2 weeks after we receive the request.
